A mandatory requirement is that the responding firm or entity must be duly licensed by the State of New Jersey to provide professional medical services, and this licensure must remain in good standing throughout the entire contract term. Beyond licensure, stringent insurance requirements include Worker's Compensation Insurance in full force, General Liability Insurance with limits of not less than $1,000,000 per occurrence and $2,000,000 aggregate, Automotive Liability Insurance for owned, hired, and non-owned vehicles with limits of not less than $1,000,000 per occurrence and $1,000,000 aggregate, and Professional Liability Insurance and/or Malpractice Insurance with limits of not less than $2,000,000 per occurrence and $5,000,000 aggregate. Certificates of all required insurance must be from a company authorized to do business in New Jersey and must name the City of Bayonne as an additional insured. The successful contractor will also be required to indemnify, defend, and save harmless the City from any claims arising from the contractor's actions.Sealed responses and proposals must be submitted to the City of Bayonne Law Department, Room 15, located at 630 Avenue C, Bayonne, New Jersey 07002.
