Employee Assistance Program (EAP) Mental Health Services

The City of Bayonne, New Jersey, is soliciting qualifications and proposals from professional service providers for Employee Assistance Program (EAP) Mental Health Services to serve its municipal employees. This Request for Qualifications/Proposals (RFQ/RFP) seeks contractors to provide comprehensive mental health services, demonstrating a thorough understanding of EAP program processes and proven experience in delivering such services to public entities. The scope of work encompasses all necessary EAP mental health support for City of Bayonne municipal staff, with the intent to award a contract for a period not to exceed one year from the contract date. This initiative is conducted under a "fair and open" contracting process as defined by the New Jersey Local Unit Pay to Play Law, N.J.S.A. 19:44-20.4 et seq. Prospective contractors must meet stringent qualification requirements to be considered for this professional services engagement. Mandatory qualifications include licensure by the State of New Jersey to provide mental health services, with said licensure remaining in good standing throughout the contract term. Firms must also possess a minimum of ten years of experience in providing EAP mental health services specifically to employees of public entities. Additionally, respondents are required to maintain a bona fide principal office within the State of New Jersey and demonstrate sufficient staffing capacity to deliver all necessary EAP mental health services effectively. Successful contractors will be required to maintain comprehensive insurance coverage, including Worker’s Compensation, General Liability ($1,000,000 per occurrence, $2,000,000 aggregate), Automotive Liability ($1,000,000 per occurrence, $2,000,000 aggregate), and Professional Liability/Malpractice Insurance ($1,000,000 per occurrence), naming the City as an additional insured. Sealed responses and proposals must be delivered to the City Law Department, 630 Avenue C, 1st Floor, Room 15, Bayonne, New Jersey 07002, on or before January 16, 2024, at 10:00 a.m. prevailing time. Proposals will be publicly opened at 12:00 p.m. prevailing time on the same date in the Law Department Conference Room, Room 15. Required submission documents include the standardized Professional/Consulting/Extraordinary Unspecifiable Service RFQ/RFP Submission Forms, which comprise a signed Proposal, Qualifications and Costs Submission Form, a signed and notarized Non-Collusion Affidavit, a signed Disclosure of Ownership form, a Professional/Consulting/Extraordinary Unspecifiable Service Entity Information Form, a current NJ Business Registration Certificate, a Certificate of Good Standing from the NJ Division of Revenue and Enterprise Services, and a Disclosure of Investment Activities in Iran Form. A detailed cost proposal, outlining fee structures (contract/lump sum and/or per patient/service) and anticipated reimbursable costs, is also mandatory.
