Please read the entire document. By ﬁlling out the application you are taking responsibility 
for the Event.  
Fields to be ﬁlled in:  
Event Date(s): If you are having a one day event, the “Start” and “End” date will be the 
same.  
Name of Organization, Business, Entity or Individual: The organization that is planning 
and hosting the event.  
Address/City/State/Zip: The oƯicial address for the organization that is planning and 
hosting the event. 
Email: The oƯicial email address for the organization that is planning and hosting the event. 
Phone#: The best number to reach your organization.  
Is this group a non-proﬁt or not-for proﬁt organization?: If you are a non-proﬁt or not-for 
proﬁt organization you must also attach the letter you received from the IRS (IRS Letter 947) 
or State of NJ [NJ Dept of the Treasury, Division of Revenue and Enterprise Services, 
Certiﬁcate of Inc, (Non Proﬁt) letter] conﬁrming your status.  
Event Organizer: Your name. 
Cell Phone: Your cell phone number.  
Title of Event: The event name you will be using on your promotional material (ﬂyers, social 
media posts, etc.)  
Event Type: Check One Only. Full deﬁnitions for each event type can be found in the 
ordinance as well as the “Types of Assemblies Covered by the Ordinance” fact sheet.  
Location of Event (w/ cross streets): Where your event is taking place. If your event is 
taking place in a park, cross streets are not necessary. If your event is taking place in the 
street, such as a parade, list the street you will be marching on, as well as the beginning 
and ending street (ex: Broadway from 5th Street to 39th Street).  
Start Time/End Time: This is the time that your event will open to the public.  
Rain Date: If your event is rain or shine please write “NA”. Rain dates are subject to 
availability.   


Alt. Location: If your event is rain or shine please write “NA”. If you have a backup location 
in the event of inclement weather, please list it here.  
Set-up Time: This is the time, prior to your event opening to the public, that you will begin 
setting up your event. If your event is taking place on a street, this would be the time that 
the street will be closed.  
Breakdown Time: This is the time you will be done, after your event ends, cleaning up and 
breaking down your event equipment. If your event is taking place on a street, this would be 
the time that the street will reopen to traƯic.

Projected Attendance: The number of people that you expect to attend. 
Prior Year’s Attendance: The number of people you believe attended the previous year. 
Please use your best estimation.  
Event Description, in detail: Please describe your event. What do you hope to 
accomplish? How do you plan to do it? Will there be food? Music? Mechanical or Inﬂatable 
Rides? A prayer service? Comedians? Animals?  
Moving Assembly: This is a parade or procession/walk. If you will not have a moving 
assembly, please skip to Will any high-proﬁle people attend this event? 
Total # of Groups: For Moving Assemblies only. The number of individual groups you expect 
to march/walk with you.  
Total # of Marchers: For Moving Assemblies only. The total number of marchers/walkers 
you expect- add up the members of each group for the total. 
Will this Moving Assembly include Floats and/or Automobiles?: For Moving Assemblies 
only. If yes, list the count for each group (Floats/Pushcarts/Motorized Vehicles- 
cars/trucks) on the line below. 
Parade marchers and ﬂoats will begin to assemble at: Time/Location: The time that you 
begin assembling should be LATER than the time you have listed above for street closure. 
Name the block(s) or other area where your marchers and/or vehicles will meet and 
organize.  
Moving Assembly will ﬁnish at: Time/Location: The time that you ﬁnish should be 
EARLIER than the time you have listed above for street reopening. Name the block(s) or 
other area where your marchers and/or vehicles will disperse. 


Will any high-proﬁle people attend this event?: List any celebrities, even local ones that 
you expect to attend.  
Private Event: Are tickets or registration required to attend your event?  
Open to the Public: Can anyone come to your event?   
Entry Fee: Will you be charging people to come to your event? 
Site Plan: This is a drawing or computer image of how you plan to set up your event. It can 
be as simple as pencil and paper or a picture from Google Maps or as elaborate and 
detailed as you would like.  
OƯicial Government Identiﬁcation: Your driver's license or other government issued ID.  
Insurance: For questions about the required insurance, please review the Insurance 
Information document located on www.bayonnenj.org/events. Your permit will not be 
issued with proper insurance.

Will alcoholic beverages be oƯered for sale?: Please keep in mind that there are 
additional steps and permits if you are planning to have alcohol. Visit the “Alcohol” section 
of www.bayonnenj.org/events for more information.  
Will food be oƯered at this event?: If you will be oƯering food it must be from properly 
licensed and inspected providers. You must ﬁll out the Health Department Event 
Application, which can be found at www.bayonnenj.org/events and submit it to 
healthdept@BayNJ.org or in person to the Health Department at City Hall (630 Avenue C, 
Room 11). Each food provider will be required to ﬁll out additional forms that you will need 
to review before submitting to the Health Department.  
Will you need DPW services, e.g. garbage removal, personnel?: There are very few times 
when an event will not require garbage removal or personnel, especially if it is open to the 
public and/or taking place in one of the City’s beautiful parks. If you are not sure, please 
contact the Events oƯice at events@BayNJ.org or by calling 201-858-6085.  
Will entertainment such as Bouncy Houses or Rides be present?: All rides must be 
licensed with the state and inspected by Bayonne Fire Prevention.
