Business Registration Certificate. Respondents must maintain Professional Liability insurance with minimum limits of $1,000,000 per occurrence and $3,000,000 aggregate. The Qualification Statement must detail the firm’s business organization, ownership structure (including any 10% or greater stockholders), years in business, current management, any professional malpractice judgments or bankruptcy proceedings, and confirm appropriate federal and state licenses. A highly recommended Pre-Proposal Conference is scheduled for Thursday, April 11, 2019, at 11:00 AM, to be held in the Municipal Council Chamber at 44 City Hall Plaza, East Orange, NJ 07018.
Sealed RFP responses are due on Thursday, April 25, 2019, at 11:00 AM, in the Municipal Council Chamber, Municipal Building, 44 City Hall Plaza, East Orange, New Jersey 07018. Submissions must include one (1) original and four (4) copies, enclosed in a sealed envelope clearly marked “HEALTH INSURANCE BROKERAGE SERVICES” and addressed to Lisa L. Jackson, Q.P.A., Division of Procurement, City of East Orange, 44 City Hall Plaza, East Orange, NJ 07018. A non-refundable fee of $25.00 is required for RFP packets picked up on-site, payable to the CITY OF EAST ORANGE. Proposals must remain firm for sixty (60) days after the submission date. Required submission forms include Non-Collusion Affidavit, Stockholder Disclosure, Insurance Requirement Acknowledgement, EEO/Affirmative Action Compliance, Americans with Disabilities Act compliance, Certificate of Employee Information Report, Business Registration Certificate, Professional Service Entity Information Form, Qualification Affidavit, Submission Form, Acknowledgement of Receipt of Addenda, Disclosure of Investment Activities in Iran, Statement of Indebtedness, Letter of Intent, and Letter of Qualification.
