Submitting firms are mandated to comply with specific New Jersey statutes, including N.J.S.A. 10:5-33 et seq. (Affirmative Action), N.J.S.A. 10:5-31 et seq. and N.J.A.C. 17:27 et seq. (Contract Compliance and Equal Employment Opportunities), and N.J.S.A. 52:25-24.2 (Disclosure of Ownership). Required documentation includes an executive summary, an executed Letter of Qualification and Letter of Intent, detailed firm business organization and ownership information, a statement of affirmative action compliance, firm history, disclosure of any judgments or bankruptcy within the last ten years, and copies of all appropriate federal and state licenses, including a New Jersey Business Registration Certificate and Certificate of Authorization. Professional Liability Insurance with minimum limits of $1,000,000 per claim and $1,000,000 aggregate per policy period is also a mandatory requirement. Statements of Qualifications must be submitted by Tuesday, November 10, 2020, at 11:00 a.m. prevailing time. Submissions must be securely enclosed in a sealed envelope, clearly marked with the consultant's name and the title "ENGINEERING & ARCHITECTURAL CONSULTING SERVICES AND SURVEYING SERVICES," and addressed to the Purchasing Agent, City of East Orange, City Hall, 44 City Hall Plaza, East Orange, NJ 07018. Firms may submit proposals via mail, by placing them in the designated "Drop Box" in City Hall’s rear parking lot, or by presenting them at the public opening. A non-refundable fee of $25.00 applies for physical RFQ packets obtained on-site, payable to the City of East Orange. Each submission requires one original set, five copies, and one electronic disk (CD or DVD). Firms must ensure all eighteen mandated forms, including a Non-Collusion Affidavit, Disclosure of Ownership Form, and various compliance acknowledgments, are fully completed and included.
