Political Party Affiliation Declaration Form

This document is a Political Party Affiliation Declaration Form for the State of New Jersey, issued by the Division of Elections. It allows registered voters to declare affiliation with a political party (Democratic, Republican, Conservative, Green, Libertarian, Natural Law, Reform, U.S. Constitution, Socialist Party of New Jersey) or to declare as unaffiliated.
