Application for Boards

This document is an application form for individuals interested in serving on advisory boards and commissions within the Township of Hamilton. The application requests personal information, including name, address, phone number, and email, as well as information on education, volunteer experience, work experience, and civic involvement relevant to the desired board or commission appointments. Applicants are also asked to disclose any criminal convictions that may impact their ability to serve.
