--- Document: Parent/Guardian Code of Conduct Document --- PARTICIPANT AND GUARDIAN CODE OF CONDUCT The Township of Lyndhurst Department of Recreation has adopted the following Participant and Guardian Code of Conduct. This Policy applies to all Participants, Guardians and other Spectators attending any Township of Lyndhurst Department of Recreation sponsored event or program. I will show good sportsmanship at all times. I will respect my teammates, opponents, coaches, officials, and all others attending any Township sponsored events. I will listen to coaches, officials, or Township Department of Recreation employees or volunteers. I will compete fairly and try my best at all times. I will support my teammates and encourage them to succeed. I will not engage in fighting, horse play, teasing, physical contact or other disruptive behavior during any practice or game. I will not argue with any coach or official's decision or publicly question his/her judgment or integrity during any practice or game. I agree to wait a full 24 hours before contacting a coach to discuss his/her decision or action taken during any practice or game. As a guardian of any player, I will communicate respectfully with the coach to determine his/her expectations and to discuss any concerns or questions I have. I (and my guests) will refrain from entering the field of play unless required to do so because of injury to my child. I (and my guests) will refrain from coaching my child or other players during a game and/or practice unless I am an appointed coach to the team. I (and my guests) will refrain from booing, taunting, teasing or showing disrespect to any participants. All communication will be in the form of support and encouragement. I (and my guests) will show good sportsmanship at all times. This means I will not participate in nor encourage any swearing, foul language, fighting, or disrespect toward coaches, officials, players or parents. I (and my guests) will not post pictures or videos that contain any participants (other than my own child) without express permission from the other participants’ legal guardians. I will be responsible for supervising others that I bring to the game and/or practice. I will not leave a child at game and/or practice unless a Township approved volunteer is present and I will ensure that the approved volunteer has my contact information. I agree to pick up my child from practice or a game at the designated end time. I agree to pick up my child from practice or a game prior to the designated end time in the event of inclimate weather, early termination of the event or cancellation of the event. I understand that Township sponsored recreation games/practices do not provide babysitting for my children. I (and my guests) will not attend or participate in any event if I am (or my guests are) exhibiting symptoms of Covid-19 or reside with anyone exhibiting symptoms of Covid-19. I (and my guests) will agree to follow all recommendations and guidance from the CDC, State, County, leagues and other local agencies concerning Covid-19 and organized sports including recommendations pertaining to social distancing, face coverings and contact. I understand and agree that a health questionnaire may be required for my child to participate in any events. I understand and agree that my child may be required to submit proof of a negative Covid-19 test upon request. I will demand a sports environment for my child/player that is free from drugs, tobacco, and alcohol. I (and my guests) will not use or be under the influence of drugs or alcohol while at a youth sports event and I will not attend, coach, officiate or participate in a youth sports event while under the influence of drugs or alcohol. I will teach my child/player to play by the rules and to resolve conflicts without resorting to hostility or violence. I will demand that my child/player treat other players, coaches, officials, and spectators with respect, regardless of race, creed, color, religion, sex ability or any other protected class. I (and my guests) will praise my child/player for competing fairly and trying hard, and make that child feel like a winner every time. Attendance and participation in any Township sponsored sports event is a privilege and not a right. Any player, coach, official or spectator's violation of the Code of Conduct policy may result in the revocation of that individual’s eligibility to participate and/or be present for any Township sponsored sporting event. I understand that the game will be stopped by the official when a parent, spectator, or participant displays inappropriate and/or disruptive behavior which interferes with the progress of the game as determined by the official or coach. Officials and coaches have the authority to remove any violator from the spectators viewing area and/or game area. Once removed, play will resume. Lost time will not be made up and violators will be subject to further disciplinary action by the Township of Lyndhurst Department of Recreation. Inappropriate and disruptive behavior shall include but is not limited to: challenging officials, inciting verbal altercation, taunting, cursing, fighting, threatening physical violence, destruction of equipment and/or property. Disciplinary action may be imposed by a Coach, Official, or the Township of Lyndhurst Department of Recreation. Disciplinary action includes but is not limited to: verbal or written warning; ejection; suspension; benching; removal; game forfeiture or any other action deemed necessary. ACKNOWLEDGMENT I, __________________________, as parent and/or guardian of minor child, ____________________, acknowledge that I have received and read the Lyndhurst Code of Conduct for Participants and Parents and that I and my participant child will abide by it. I have discussed the Lyndhurst Code of Conduct for Participants and Guardians and the Township’s expectations with my child. I acknowledge that any violation of this Code of Conduct may subject me, my child, my guests, and other family members and spectators to Disciplinary Action. I further acknowledge that Disciplinary Action is not progressive and is made on a case by case basis in the sole discretion of the coach, official or Township of Lyndhurst Department of Recreation. Any objection to disciplinary action taken must be made to the Recreation Coordinator. Parent/Guardian Signature:_________________________ Athlete/Participant Signature:_________________________ Date: __________________________ WAIVER, RELEASE AND INDEMNIFICATION AGREEMENT I, __________________________, on my own behalf or as parent and/or legal guardian of minor child, ____________________, acknowledge and agree as follows: I understand and acknowledge that I or my child can suffer bodily and other injuries at any time and without warning while participating in programs or events offered by the Township of Lyndhurst Department of Recreation. I understand that anyone participating in any sports/fitness program should consult a physician before beginning any program. The Township of Lyndhurst and/or its agents, servants and/or representatives have made no evaluation or recommendation as to my or my child’s ability or fitness to participate in any programs offered by the Township of Lyndhurst Department of Recreation. By participating in programs offered by the Township, I acknowledge that my child is fit and able to participate in the Program. I have reported all known allergies and medical conditions to the Department of Recreation and any volunteers or coaches. I understand that the Township of Lyndhurst is making all reasonable efforts to comply with the considerations, recommendations and guidance from government agencies concerning COVID-19 and organized sports. I acknowledge and understand that anyone participating in the sports programs may be exposed to or may contract COVID-19 during their participation. I further understand and acknowledge that my child may be required to submit to temperature scans and complete health questionnaires before being permitted to participate in any practice or competition. I understand that the Township of Lyndhurst may change the requirements for participation as the State recommendations and guidelines change. I understand these risks and I agree to assume each of them. I hereby RELEASE AND DISCHARGE the Township of Lyndhurst and its employees and volunteers from any and all liability, claims, demands or causes of action that I may hereafter have for any and all injuries or claims for damages arising out my or my child’s participation in any events sponsored by the Township of Lyndhurst Department of Recreation. I also agree to indemnify and hold harmless the Township of Lyndhurst, its agents, employees, volunteers, representatives, independent contractors, and/or assigns against any and all liability, claims, damages, costs and expenses, including reasonable counsel fees, which arise out of, result from, occur during or are connected in any manner to any action, accident or incident arising out my or my child’s participation in any events offered by the Township Department of Recreation. This waiver, release and indemnification agreement shall be construed pursuant to the laws of the State of New Jersey and that New Jersey courts, whether state or federal, will have jurisdiction over any matters arising from this document.
