Bingo Application

This document is an application for a Bingo License from the New Jersey Office of the Attorney General, Division of Consumer Affairs, Legalized Games of Chance Control Commission. The applicant organization must provide details regarding the organization's name and address, the dates and hours of the games, the address where the bingo games will be played, and a schedule of expenses and purposes. The completed application, along with the applicant's registration slip, must be submitted in four copies to the Municipal Clerk’s office in the municipality where the games will be conducted.
