Vote by Mail Application - English

This document is an application for a Vote by Mail Ballot. Registered voters can apply to receive their ballots by mail for a specific election or for all future elections. The application requires personal information such as name, address, and date of birth, along with the applicant's signature. Completed applications must be received by the County Clerk either by mail (postmarked no later than Election Day and received within 6 days) or in person, no later than 3 P.M. the day prior to the election.
