Citizen Volunteer Board Application

This document is an application form for citizens interested in volunteering for various boards, commissions, or committees within the Township of Upper. Applicants are required to provide their name, preferred committees, relevant background and training, and disclose any prior service on Upper Township boards or committees. The application form specifies that all submitted materials, including the form itself, may be considered public records subject to disclosure under the New Jersey “Open Public Records Act,” with the exception of certain personal information explicitly marked as confidential. Completed applications should be returned to the Upper Township Municipal Clerk.
